I'm a special education parent advocate. One of the children I advocate for has a severe emotional problem that requires medication. After his mother noticed that he had developed a temperature and was sleeping a lot she called the psychiatric hospital from which he had recently been discharged. Turned out that one of his prescriptions was incorrectly filled by their pharmacist (wrong med - huge dose).
