As I have mentioned, Will Cope (Willamette-Columbia Parkinson Society) of Oregon has a hospital med info sheet to take with you to the hospital. Call 1-800-7384 for a copy of this simple piece of paper--also there is a sample print out. You can make a few copies and have one filled out in case of emergency. Be ready. Helen Mason
