Jane...About 15 years ago I had a friend whose HMO backed out on spinabifida
surgery for her young daughter...and another whose Blue Cross rejected a
gall bladder surgery payment saying it was a pre-existing condition ...they
fought the two insurance companies and got no where until they got the State
Dept. of Corporations involved...shortly after that both insurance companies
paid in full with no more problems.  You might want to try your State
Dept.of Corporations which oversees all corporations and if it's
incorporated in another state, notify that state's Dept. of
Corporations...good luck.  Joan H.
