There's a "'rejuvenating service" which I THINK is owned and operated by a plastic surgeon(s), that advertised on the talk-radio station I listen to. Amongst the huge variety of cosmetic-type procedures and services they offer are three botox injections for $100. One could schedule the injections whenever they wanted to have 'em. I'm presuming this is some kind of boiler-plate operation that deals almost exclusively with cosmetic type procedures. Sorry I don't have a name or phone number for the service, but I wasn't interested in it it at the time the advertisements ran..
