Dear Wendy, Usually up to 5 or 6 tablets of Sinemet 25/250 is the maximum dose. For patients who had a pallidotomy or pallidal stimulation dosages up to 10 tablets a day can be given, becuase the pallidotomy takes care of the dyskinesias. To improve the dyskinesias in your mother higher dosages of Sinemet or Madopar should be avoided and if not already done, the dosage should be reduced and a dopamine agonist (Permax, Parlodel, Requip, Mirapex) added. Or one can add a COMT-inhibitor like Tasmar or Comtan/Comtess to improve the fluctuations. I am afraid the medication adjustment may take time, a lot of patience and a dedicated physician.
