Out where you can't see them from a rooftop are the recipients of NIH grants for research. Smaller labs struggling in their own way, but, possibly, more strongly motivated (publish or perish doesn't quite do it justice) and more focused. Some of the latest NEWS: items seem to be coming from the drug companies and universities. And, recently there is news of funding some Parkinson "centers", each with its own unique contribution. I also think that the NIH initiative for electronic publication of research (with both positive and negative results) is a massive step in the right direction. It will get info out there faster and cheaper and possibly to more viewers. Couldn't see that from the rooftop.
