Comtan (entacapone), now available in the U.S., is supposed to smooth out the response to levodopa. How does one take it in conjunction with Sinemet to achieve the optimum effect throughout the day? There are a few variables to consider: 1. With regular Sinemet, the concentration of levodopa in the blood stream rises, then falls, sharply. 2. With Sinemet CR, the concentration of levodopa in the blood stream rises, then falls, gradually, over a longer time period. 3. Comtan has its own cycle of rising, then falling, concentration, for a different duration, through which it will have an increasing, then decreasing, effect of prolonging the levodopa concentration level. 4. Then there are meal times, during which, depending on the quantity of food and the fat and protein content, levodopa delivery is delayed. How do you figure this out? My head is spinning.
