Hi Brian.  I, too, have been experimenting with a 'shot' (sinemet)at 4 a.m.
or even earlier whenever I first wake up.  So far (3 nights in a row) there
has been very little
relief.  My dystonia has developed in the shoulder/neck, left side.  Began on
Tuesday the l6th at 4:30p.m.  and continues at this time.  It abates for a few
hours to a tolerable ache, then builds to a crescendo.  This building up is
another cycle I'm tracking on a chart to pin down exactly where I am in my
dosing.  Coincidentally, I am in the process of switching over from SinemetCR
25/l00 to regular sinemet 25/l00 , a plan which was expected to eliminate the
mid-afternoon dyskinesia.  That expectation has been realized.  But also,
to complicate the diagnosis further, I didn't get the regular sinemet from
the drug
store until Wednesday, the l7th, so I  believe that is significant.  Boy, one
should be a rocket scientist to sort out all this stuff.  In the meantime the
ache/pain goes on and on.

I've been trying to get a TENS unit, with many obstacles such as Medicare
and Secondary and prescribing doctor, etc., etc., and feel next week may
turn up some relief.  I hate to ask my doc for a muscle relaxant until I have
tried this TENS.   Have you ever used one?  Do you have any suggestions for
me, Brian?  I am so INTO this problem that I can't think straight.
